Central Baptist Hospital Home Health Care offers home-based diabetes education and self-management support for people with diabetes.

Our RNs offer diabetic education that focuses on our patients' specific needs. A dietitian is also available for nutrition counseling.

Registered nurses are available to assist you 24 hours a day, seven days a week. You may call our office or have your physician contact us. Physician referral and a homebound status are required for the program.
