Accessing Home Health Care Services

Your physician must order home health care services for you. You may call our office and we will call your doctor to obtain an order for our services. If desired, your physician can call our patient care coordinator or use Home Health At Your Fingertips, a special form that allows doctors to refer patients to Central Baptist Home Health Care on-line.

A registered nurse will work closely with you and your doctor to establish a treatment plan. A nurse is on call 24 hours a day, seven days a week.

Our staff members will help you determine the home care services covered by your insurance plan. Payment sources can include:

• Medicare;
• Medicaid;
• Private insurance;
• Self pay;
• HMO'S.
