Documentation requirements vary by area of practice, but include specialty certifications, copy of Degree, applicant photograph, liability insurance, and delineation of privileges. The applicant’s educational training, work history, liability history and professional references are verified directly with each source. Application verification responses may take several weeks to receive from the individuals, sometimes delaying the process. The applicant can assist in this gathering of information by contacting the individuals at their respective school(s) and previous employment, and by notifying references of the evaluations being sent to them. Ensuring receipt of all required documentation is the responsibility of the applicant.
Once the application is complete including all required documentation and returned verifications, it is reviewed by the appropriate individuals and forwarded to the Credentials Committee and Medical Executive Committee for recommendation, prior to action by the CBH Administrative Board of Directors.
