Personalized Home Health Care

Our team of healthcare professionals will work with you and your physician to develop a plan of care that meets your individual needs, which may include: home health nurses, home health aides, physical therapists, occupational therapists, speech-language pathologists, medical social workers, chaplains and pharmacists.

Accessing Home Health Care Services

Your physician must order home health services for you. You may call our office at (859) 260-6569 or toll free at 1-800-489-3002 and we will call your doctor to obtain an order for our home health services.  If desired, your physician can call our patient care coordinator or use Home Health At Your Fingertips, a special form that allows doctors to refer patients to Central Baptist Home Health Care on-line.

A registered nurse will work closely with you and your doctor to establish a treatment plan. A nurse is on call 24 hours a day, seven days a week.

Our staff members will help you determine the home health services covered by your insurance plan. Payment sources can include:

  • Medicare
  • Medicaid
  • Private insurance
  • Self pay
  • HMO'S
